We had a family member cared for at this facility after a stent in the hospital. We were really pleased with the help that they received, and felt like the staff was really prompt in meeting their needs in a timely manner, which was huge for our family, being that this family member required quite a bit of attention. Overall we had a great experience here and would recommend to others needing a place for their loved ones.

Had a relative placed in this building and have been kind of iffy on how good the care is. But one person who stuck out was a therapist assistant named Brook… she was compassionate and seemed to do her job very efficiently providing the care that any building should give.

The experience we had here was repulsive. The first 3 days he was not dressed or cared for even though he had a bag of clothes. I walked in on day 3 with him laying in bed with just an adult diaper on. Hair not combed, teeth not brushed. Nothing. We could never get through on the phone to check on his status. Many of the voicemails are full and you cannot even leave a message. If you could leave a message, you did not receive a return phone call. Come time for discharge, they called around 6 pm Tueday letting us know and if we wanted to appeal to insurance it had to be complete by noon the following day. NO NOTICE. Upon discharge we had to fight to get the needed paperwork completed and they wanted to place all the blame for the incompetence elsewhere. STAY AWAY!

I had mom placed here two different times. Once to be rehabilitated after she had a stroke. She lost all her abilities to function. Speech, walking, the whole nine yards and re-gained them all back with the wonderful rehab team.
Five years later, this year, she was back again and they took amazing care of mom once more. Her entire team of caregivers, Dr’s, speech therapists, rehab specialists, case manager and everyone there worked hard to make sure she received the care and rehabilitation that she needed.
Unfortunately she was not well enough to come home this time. I absolutely believe in my heart of hearts I could not have placed her in a better care facility to live out her last months had I tried.
Everyone there treated mom and myself as family. I returned to tie up some business and everyone remembered me and asked how I was doing after losing mom. I will truly miss all of the amazing care team at Boswell Transitional Care of Cascadia. They all are amazing people!

I am sharing this exact review on Yelp so that there is no inconsistency in my feedback
Would be zero stars if possible. My 81y mother took a fall, stabilized at Banner Del Webb Hospital for 2.5 weeks and transitioned here. She experienced cognition issues after the fall that didn't abate. I am her son and Medical POA
Bottom line - they are not remotely equipped, at a minimum, for dealing with even minor delirium or cognitive issues.. They didn't give PT until 5 days after checkin. After demonstrating agitation, they literally knocked her out with Ativan, didn't check her catheter or urine for at least 3 days and then added 2 more doses of the Ativan benzo class. After 6 days, she was taken by ambulance and admitted to the hospital: bladder distended and overfull, in sepsis, with pneumonia, with a full UTI and candida.
She checked in on a Thursday evening and facilities are OK.
- Friday - no visits from OT, PT, Case Management
- Saturday - brief visit from OT - did BIMS assessment, which was not shared until a friend asked if they had done one.
- Sunday - no therapy, got out of her bed overnight, found her on the floor
- Monday - she began showing agitation, they moved her into the hallway to monitor. No OT or PT. A psychiatric Nurse Practitioner conducted an assessment sometime on Monday (I was there 8-2 and 4-6), but no one told me even upon asking. Gave her Ativan overnight. No indication from nurse even though asked med status
- Tuesday - continued agitation, No OT or PT. kept her in the hallway. Then, gave her Ativan (Xanax family benzo) at 6p and then 11p. No phone call. I did not/would not have OKed that class of drug.
- Wednesday - arrive 12noon, she was out cold, PT first assessment was at 1pm, she could not be roused. PT later wrote that she was non-participative. Well, turns out 3 does of Ativan does in 24 for a 100lb 81y will have the effect of knocking her out. Asked for psychiatrist ASAP. Called 1h later by "Emilie" who said "I should have been called and I apologize" and that "Ativan was appropriate given her agitation." She subsequently stated there were notes that she was aggressive, attempting biting a nurse and other feedback - none of which was ever shared with me.
- Wednesday night 7pm - call indicating she was taken by ambulance to hospital. ER says she has sepsis, pneumonia, UTI and was "overfull urine" - that it appears her catheter was kinked for "an extended period of time." She did not wake again until Friday 11am.
I will be seeking legal feedback once my mom departs the hospital and gets stable.

I have been in Boswell Transitional Care for over a month.

Recreation Department
These departments do very well professionally and the staff are very personable.
IF complaints reach the *Ombudsman* (client advocate… outdated 1/3 of the page near the back of Boswell’s Welcome Booklet) they will visit Boswell and speak with a number of clients and presents those complaints to Management and will monitor the facility for a time.)
Cons:
This facility does NOT request information from clients Primary Care Providers (list of medications, allergies, and known medical conditions).
The nursing staff breaks orders of supervisors & upper management (I’ve witnessed them openly going against these orders).
The social worker and many nursing and kitchen staff are verbally combative, even RAGEful, with us clients/residents. It’s even worse with Alzheimer’s residents.
Staff are not respecting Guardians requests and are not allowing guardians to speak to management in their client’s behalf.
Clients have to repeat requests for hours or even days (medication, supplies, doctor’s visit, management, etc.).
Staff ignores requests for a different nurse/cna. (Residents are clients and have the legal right to fire their Nurses, CNAs, and Management.)
Nurses are not informing their clients of what medications they’re on and they are not reporting side-effects.
It’s ‘standard practice’ to give lethal combinations of medications to clients. (I.e. Pain relievers & muscle relaxers cannot be mixed with opioids; all 3 are central nervous system depressants and create a very high risk of the lungs shutting down.)
A number of us residents get our medication 1-3 hours after the scheduled time and/or after requesting it.
Most of the nurses put the medication on the table and walk away.
Some nurses tell housekeeping to turn off the call light without seeing what the client needs.
Nurses work with equipment that is malfunctioning.
Nurses have to run to other wings for supplies & medications.
Staff does not respect diabetics’ remote monitoring. There is no dietary protocol for diabetics, (not even the alternatives menu). Staff is combative when a diabetic asks for insulin because their numbers are too high. They’re also sloppy with the insulin dosing.
They keep giving Miralax (heavy duty stool softener) to us residents who don’t need it and/or to ones who can’t have it due to a severe allergic reaction.
They do not have appropriate supplies for bedridden clients, not even leg compressors, which puts residents at a high risk for blood clots (clots lead to limb amputation, heart attack, and/or stroke).
There is a covid outbreak. Nurses refuse testing and will not tell the doctor until after the incubation period is over; we have to fend for ourselves. The staff is unnecessarily exposing a number of other caretakers and visitors to covid. The facility does not have warning signs posted. They’re still bringing in a ton of new clients.
The disability button to the patio door is broken. Also, clients rooms have things broken.
I tried to check myself out, but couldn’t. If I check out against doctor’s wishes my insurance will not pay the bill; I will be made to pay it. They have in house doctors, labs, x ray technicians, psychiatrist, therapist, Alzheimer’s ward, etc. If a client/resident is being mistreated, it stays ‘in-house’.

It has taken me over a year to recover from the trauma of being in this place for 3+ months. It was the worst place I’d ever experienced. I still deal with many issues associated with my stay. I simply cannot relive it by providing details in this review. I will simply say that it was horrible and if you can go somewhere else or place your loved one elsewhere you would be well served. Wishing you the best of care at a much better facility.

My husband was sent here on Monday. It is now Wednesday and he still has not been seen by a physical therapist, which was the reason why he was sent here for PT. On Monday when he arrived, I told the nurse the bathroom had fecal matter all over the toilet from his roommate she said she would clean it up, she did not. On Tuesday the fecal matter was all over the toilet and a nurse came in and we ask her to clean it. She would look at it said oh. i’ll clean it up but she never came back to clean it up. Living in such unsanitary conditions I am afraid my husband is going to get E. coli or some dreadful disease because his roommate is just nasty. Today is Wednesday and I think the only reason they clean the toilet was because my husband dropped his toothpaste and container. You keep the toothpaste in in the toilet. so they had to clean it up then. On Monday evening, the student who was taking my husband’s blood pressure evidently did not know how to work the machine because it never read his blood pressure and the guy just took it off his arm and left. It’s a good thing. My husband doesn’t have high blood pressure because having help like this he could’ve died, from incompetent help like this. Today they told my husband he couldn’t get physical therapy because they were too busy and far behind because of the holiday. Well, if you don’t have enough staff to take care of your patients, then you should not have accepted my husband to this facility. I was going to take a picture of the nasty toilet to post but since it got clean today after three days, I wasn’t able to get the picture.
