Canyon Oaks truly went above and beyond in caring for my mom. From the very start, Ruby and the entire case management team were so kind and helpful, making the intake process smooth and stress-free. The nurses were incredibly responsive—Alan, in particular, was outstanding.

The LVNs were amazing, ensuring we had everything we needed and always working with care. Maria is absolutely extraordinary.

The food was fantastic, and the head chef even personally went over dietary needs to make sure everything was perfect. Such a thoughtful touch!

Andre in Physical Therapy is nothing short of a miracle worker—he got my mom walking again after 10 years. It was truly incredible to witness.

Dr. Levi was so smart, caring, patient and takes his time with you and easy to work with. I wish he could be our primary doctor all the time!

The facility itself is clean, and the staff are genuinely fantastic. I can’t say enough good things about Canyon Oaks. They gave my mom the best care possible, and I’m so grateful. Highly recommend!

Everyone at Canyon Oaks have been great. Whenever I call to talk with my mother or the nurses station there is never a problem. One of the nurses helped me label my mother's clothes I had brought from home. I can't remember all of the nurses names but Jose is very helpful and full of information. The place is always very clean and patients all seem very happy when I am walking through the entrance to the building. 5 stars

Canyon Oaks is the best facility around. Everyone is so nice and you feel a sense of welcoming when you walk in. I felt included in my grandmother's care plan and I know she's in good hands there. My grandma told me she feels very comfortable and that's a big deal to me. If you or a loved one needs care, I highly recommend Canyon Oaks.

My mom has had two stays here rehabbing from hip issues. Supportive staff. Communication could be better but they are very busy which is understandable. She’s not too happy with the food- many people are on pain meds and the choices lean heavy on carbs and white flour items which are not great for recovery. Laundry service is very slow and hard to connect with. Front desk is friendly. I look forward to having her return home.

The staff was nice and the facility pretty. However, I feel they were negligent with my mother’s care. Their doctor only consults staff over phone and doesn’t visit. My mom’s leg turned purple and was like that all day - it took until late in the night before they got authorization to send her to the hospital (should have been immediate). She lost her leg at hospital. Maybe she would have lost it anyway, maybe not but I feel they should not have so much red tape - that hesitation could be dangerous for anyone with serious conditions. Make sure qualified family member or qualified representative who cares is with your loved one daily to monitor they are cared for with proper frequency and don’t keep them there long.

This rehab was referred to us by our case manager at Kaiser. She said it's one of the best ones near our house. We brought my mom here after her hip surgery. We are so glad my mom is here. The staff are super friendly. The nurses and CNAs are very nice, always smiling, and patient. The physical therapists and occupational therapists do a great job helping my mom recover.

My mom entered Canyon Oaks on September 13th and will come home on Wednesday October 2. For the 18 days that my mom will have been a patient, we have nothing but good things to rave about. The staff is exceptional. From the people at the front desk to the nurses, PT, OT, doctors, social workers everyone has been so helpful and kind. When your parent (or loved one) enters a SNF it is very difficult. I can’t tell you the number of times someone asked ME if I needed something while visiting my mom.
