I have been a patient at clean slate for almost 5 years. I have had at least 5 different providers. Every time my provider has been changed (without warning) my recovery would suffer as far as short pill counts, foreign substances in my system, etc. I had a really good repore with my last provider(Monica) who too was placed in a different position. She is doing Tele Health now and no longer sees any patients. She worked wonders for me in my recovery process. It would greatly benefit me and my recovery if she were put back in charge of my care. I'm sure that I am not the only patient here who would benefit by her return. If a recovering drug addict is to be successful in their recovery then their care absolutely needs to be consistent. I'm not sure who is in charge of these decisions but, I don't believe they have the patient's best interest in mind. I mean not only are we your patients in need of care but ultimately, we are your customers. I for one, would like to get better customer service.

I have been going to Clean Slate now for a couple months and I would recommend that anyone that is looking to stop using but you need some sort of help this is for you. They are very professional and only want to help, and you will never have to worry about them judging you for any reason. Give them a shoot what do you have to lose.?

Really great experience here. All the ladies working here are awesome. The front desk actually care if you get your meds or not from pharmacy.

Dr. Holder is awesome! He is compassionate and understanding of our situation as addicted people. The girls at the front desk and the rest of the staff that I have met are awesome too. These are truly caring people. I can't say enough good about them.

It went from a 5 Star rating to a 1 Star rating since they FIRED ALL of the GOOD PEOPLE in their office!!!
That place was awesome man, but now they Fired Dr. Holder, Linda, 2 Great nurses, and one of the 2 Amazing front desk ladies!!! Finally they brought one great front desk lady back, but that’s it! Evansville CleanSlate....get it together because you are making it VERY DIFFICULT for Your patients!!!

I got reminded of the horrors of clean slate today, by one of my insurance clients, battling a very high bill... Made me look back at my own recovery journey. I came to CSE in 2019. Me & my now ex husband I remember Dr holder telling us when we started to argue to call him so he could mediate it. THATS HOW MUCH HE CARED! Idk if he was fired or knew upcoming changes so he left. AMY (WE ALL KNOW HER) that "better than addicts, I will get respect or u will pay Amy" came in as a replacement. That's when my recovery became a nightmare. The first time I met her, we didn't see eye to eye. She disrespected & had a rude tone. NOBODY deserves unearned respect so I matched her disrespect with a matching tone & demeanor. No more, no less. Weeks upon weeks after that first visit with Amy, I was thrown into w/d. ALWAYS DOUBLE BOOKED ALWAYS THE ONE THEY MADE RESCHEDULE WITHOUT EVEN A FEW DAY SCRIPT TILL. This I 1000% believe was intentional. Your either amy little bow down person, or you pay. It hurts looking back knowing how many are now where I was then. When it happens to you... Call boyette, groups, even malek. Or pain clinic on East side. DONT TAKE THEIR disrespect!! You don't deserve that! Nobody does. We all deserve respect! A recovery place ain't the right spot for childish power games!!

I have seen other providers in the past and had bad experiences with rude un caring staff. However, the staff at Evansville indiana CSC is on another level. They always treat me with utmost respect. They are also always on top of prior authorizations. Whereas every other place I have been never kept on top of the prior auths so every few months I would be without meds u til the prior auth was approved. The staff here at Evansville CSC understands how important it is that the patients receive their meds. Mostly, they understand how important it is to treat the patient with respect

This was the best place to go to. Also this places saved my life . But there are people here that makes your life hell literally. Also they make you feel no important and always thanks your lying to them. But there is one or two amazing people here that don’t treat you like you like you’re a normal person. You know who you are. I will be looking for another place.
