Clean facility and live in residents seem happy. However, mom was a rehab patient here three years ago. It was a struggle to reach someone there as no phone in room. Rarely received updates and happened upon things that, myself and others, should have been made aware of. No sense of urgency from weekend nursing. Several occasions it took up to 45 minutes for a response to the call button. Need more timely response to dehydration and sepsis signs to be able to respond sooner.

If there was an option of negative 20 star review that is what i would do !!!!
Please Do NOT ever place your family members in this place if they need care. Be aware there are 3 locally. In Conroe, Tomball and Cleveland Texas. The management is poor to say the least and while there are a few good people working here there are more that do not care about the patients and do not do their job which is a big responsibility to care for vulnerable people. My mother was here and died due to their negligence in management and most especially a certain nurse named Amanda who not only failed 100% to do her job but also lied on the skilled nursing notes of care which is a felony. She stated she had given a breathing treatment an hour and a half after the patient was dead already. She did not go into the room at any time throughout the day from beginning of her shift until my mother was found dead by a caregiver. My mother laid there unable to help herself and cried out over and over for more than 3 hours to everyone near by to help her put her oxygen back on. She was ignored by Amanda who was the nurse in charge of her care as well as 3 caregivers who went into her room throughout the time on video. This is absolutely despicable. I am appalled that these people call themselves a nurse or caregiver of any kind. Please do your selves and family members a favor and do NOT place your family in this facility. They will let them go hungry, not bathe or change them properly, not give meds as per their plan of care but rather to their convenience, not call the doctor or EMs as needed, I personally came in amd found my mom unresponsive and called the EMS and found out at the hospital her blood sugar was 60 as well as she was hypoxic and overwhelmed by carbon dioxide due to lack of oxygen. Someone had turned her oxygen down to a 2 on the concentrator which I later found out by her own mouth was the director of nursing Donna who stated in front of several witnesses that she was "experimenting" to see if that helped my mother calm down and relax. The rooms are not cleaned properly, Patients were transported in a van with no working air conditioner in 100+ degree heat, there were electrical problems, flies on the beds and patients, in general it is all around a bad choice to place your family here. I again say there are a few good people however the poor management who does not listen to the patient or families and does not do their job correctly are a big problem. I complained multiple times and was basically ignored and was in the process of moving my mom from here when she died due to the nurse's negligence. Please Do Not take your family here for any amount of time. Do not take them to any of the three places owned by this franchise. I regret that I was unable to get my mother moved fast enough from this place. Please save yourselves and family from this nightmare.

Went there to visit a friend and I was impressed with a young lady by the name of Brandi. I saw that she is a person who loves her job and apparently the residents think highly of her. If you had more like her I would even consider signing up when my time comes!!! I was overall impressed with the entire facility.

I cannot say enough positive things about the care and attention the residents receive. The staff are incredibly warm and compassionate, treating each resident with the utmost dignity and respect. The facility itself is clean, well-maintained, and feels like a home. The BEST part of their facility is the activities program. Brandi makes sure everything is vibrant and engaging, offering a variety of options for residents to stay active, social, and entertained. I’ve seen firsthand how the she goes above and beyond to ensure that everyone’s needs are met to simply offering a comforting word. The activities program she has really is top notch, from the big events like parties and outings down to the daily fundamentals.

My mom was admitted to CHCC after a hospital stay for a fall. Their care for her was very comforting to my family and beneficial to my mom. She received PT and regular meals that helped build her strength and put her on the road to recovery. Many members of the staff were quite helpful. Jamie, Stacey, Bree, and Gilbert were there to communicate to me every day about Mom's care. Megan (speech) was especially helpful with managing meals and cognitive improvement. Becky at reception was the first smile we saw every day we visited and was the last one to help us out the door when Mom was discharged. Thanks to all of you.

As the Resident Council President, I feel fortunate to be part of this wonderful community. The camaraderie among the residents and staff is outstanding—there's a true sense of family here. Our activities, led by Brandy, keep us engaged and busy, which is something I really appreciate. The facility itself is always clean, well-maintained, and comfortable.
I want to especially highlight Martha, the nurse, who has saved my life twice. Her care and dedication to the residents is unmatched. She’s truly an angel, and I feel so lucky to have her as part of the team. Overall, I couldn’t ask for a better place to call home.
