When having to face the reality of placing a loved one (or having to place yourself) in a Skilled Nursing environment, no matter the length of stay, is daunting and emotionally overwhelming. Having researched facility after facility as potential placement for our loved one, I learned the harsh reality of many such facilities. Desert Mountain Care Center has opened my eyes and gave me much needed hope. Their facility is inspiring and empowering. Their physical and speech therapists including their assistants and more importantly their leader, have gone above and beyond their call of duty to help this family and our loved one overcome hurdles necessary to regain some resemblance of the life once lived. Not only have those goals been achieved but their team have enabled him to surpass the minimum required and achieved greater goals that were above the scope, leaving our loved one feeling vibrant and able once again.
The entire nursing team(RN/CNA/Assistants) are in my opinion, some of the few remaining angels who have been able to continually provide excellent care in some of the most unideal of situations. Their handling of patient care, paperwork, families and their advocates, requests, constant questions, and incesant requests were always handled with a smile, direction, and explanation. The on staff Doctors handled inquisitions quickly and thoroughly, taking time to actually include the families in treatment or care.
I fully believe in the team at Desert Mountain from the top to the bottom. Every interaction no matter the situation, was handled with empathy and we left feeling informed, heard, not alone, and that we had strength behind us if necessary.

Nursing Homes are scary and I wish that there was some sort of facilitation for families that could provide them the ugly truth about some of these facilities while also providing comfort in knowing they're not all bad you just have to know where to look, how to look, what to look for, and the questions you should always ask. An advocate who could listen and guide them through this process as they advocate for themselves and those they love. One day perhaps but for now my advice would be to do your research just as we painstakingly have, and you too will find that diamond in the ruff just as we did. *Every patient and their experience is unique and different and for us we cannot say enough positive things about Desert Mountain Care Center. This review is in our opinion based off the experiences we had.*

Words cannot express the depths of our gratitude and appreciation for everything they have done and continue to do for others. I hope we won't have to meet again but if we do I know everything will be alright.

My husband was there for a short time and they wanted to release him. I appealed with Medicare and Medicare agreed with me that he needed more physical, occupational and speech therapy. My appeal was approved on 8/23 and that same day they asked me what day I was picking him up. WHAT?…. He was no where near ready to come home but they “said” they had done everything they could for him. Sad because Medicare did not agree. The staff was friendly however when I came to pick him up on 8/25 we waited two hours for them to provide me with take home instructions. We ended up not getting any release information directly from staff just a plastic bag that contained pills and his release notes. My husband has a g tube so I must feed him. Staff gave me nothing, no feeding syringes, no liquid food, no gauze to keep area clean. It should be noted that his feeding tube was filthy and it obviously was not flushed. Nor did he have any gauze around his stomach opening where it was starting to get infected.
If you have a loved one who needs rehab services I would definitely NOT recommend this place.
I am definitely filing a grievance with Medicare because of his early release and the conditions of his g tube. Terrible….
Just as an aside I would of requested a transfer to a different facility had they kept him there past 8/25 that’s how much I disliked this place.

Worst place ever! I had to stay there for 10 days In August 2024. Most of the staff took forever answering the calls for help and if they answered it then they would come in the room and say " oh I'll be back " and leave the room , then someone else comes in and they say the same thing, finally the 3rd nurse helped me. The food was terrible and they didn't follow dietary instructions and sometimes they would not even give things mentioned on the menu for that week. Plus at night my roommate and I would get hungry since e the served dinner so early and we asked the nurses for a snack they said " we don't have any thing " but management said that thenurses stations had snacks. Also I felt nauseous one day ad the couldn't even get me a sprite or 7 up from the vending machine and they didn't have any lemons either.

My grandpa needed rehab care following a fall and hospital stay. He said the staff was excellent and he always enjoyed talking with them. He even said the food was pretty good, he had no complaints there. As a family, we do wish the staff had been more communicative with us, but the times we reached out they were very helpful. He shared a room with one other person and a shared tv. The physical space and equipment could definitely use an upgrade. It felt kind of dark throughout. Regardless of that, his experience there was positive. We appreciate all of the hard-working staff who helped him recover.

My daughter is unfortunately a disregarded patient here. I won’t go into details as I fear their retaliation on her. But when I am finally able to get her moved, I will let it all out. I wouldn’t place Satan here for treatment let alone people you love.

My husband needed some care following discharge from JFK Hospital. He went to Desert Mountain for some Physical Therapy. During his stay, the staff were wonderful & very caring. He was in a shared room with his own tv. The room had a window which could be opened for fresh air. It faced onto a pleasant patio with tables & umbrellas where patients & family could relax or take their meals.
His main complaint was about the food. He’s pretty easy to please, but he said it just didn’t taste good. He was happy to get home, of course, but the staff made his stay as pleasant as could be expected.

I fell at their facility while trying to get into a wheel chair with help of a nurse aid. I went to sit down in the wheelchair and the brakes were not locked and the chair slipped out from under me. I fell backwards and hit floor. My newly operated on left shoulder was hurting. I went to the ER then went home. No person has called to check on or given me an incident report to, fill out. This happened on Monday and today is Thursday. I told my doctors about it. They are looking into a different placement for me.

Holli Night shift CNA thank you so much for making me smile and feel comfortable at the facility. There are some good nurses here but there are also some that needs to be retrained. The facility is not perfect but with that DON I see it getting better she addressed all of my concerns. I am proud of the nurses they have a lot to do daily and at night thank you all for caring!!!!
