So grateful for the amazing care we received at Form First. Lora required PT 3x/week for 12 weeks and went from constant debilitating ankle and knee pain, due to tendonitis and misalignment, to being able to walk, jump, and eventually run again with no pain, increased strength, proper form, and newfound confidence. Kristina was so wonderful for her - knowledgeable, understanding, patient, and skilled. She made what could’ve been an unpleasant experience fun and inspiring. Lora proudly wears her Form First sweatshirts around town - they have truly changed her life! I would highly recommend them to anyone in need of physical therapy.

We first used Form First last year when my husband went through an ankle fusion. We were recommended to Nick and Kristina through another PT practice. My husband came along so quickly that he was thrilled. Nick is professional and knows how to bring his clients to the next level every week. He knows when to push further or scale it back. My husband, a year later, still speaks so highly of his PT experience. Knowing this, I knew I would not go anywhere else `after having my hip replaced a month ago. I'm so pleased with the progress that I am making with Nick in just a few short weeks. Both my husband and I highly recommend this practice. All the staff is amazing. If you want to make progress, be safe while doing it, and be greeted with a smile every time, then definitely call Form First and speak with Nick or Kristina.

Form First Physical Therapy was very clean and personable. I had a dislocated disc in my neck and was successful in avoiding surgery by utilizing the exercises taught to me at Form First. Nick was patient and knowledgeable. I would highly recommend !

I could not be happier with the services provided at Form First. The staff was absolutely amazing. Kristina worked hard to meet and exceed my goals towards my recovery. Due to the type of leg injury I had, I spent many months in a wheelchair. Kristina pushed me in my physical therapy to get back to walking normally. I had a serious problem with my shoulder and my doctor had prescribed aggressive physical therapy to help me to overcome my issues. I found the staff at Form First to be very caring and knowledgeable, and they worked closely with me to help get my shoulder back to where it should be. They also give me exercises that I could do at home to help speed up my recovery. I have already recommended them to others, and would use them again if I ever had another orthopedic issue.
