Staff was splendid. Courteous, helpful and all with a joy for life. A friend of mine is there, recuperating after a mishap at home and I sought approval from Administrative Staff for permission to bring my Support Animal when I visit. I presented the required documents and after a short discussion was admitted with a warm welcome. My friend, and Staff were overjoyed to see my dog, as were the numerous additional patients we encountered inside the facility. It is healing to my heart and soul to see how the presence of a calm animal brings such immediate joy to others while they might be struggling not so obvious issues. Thank You Horizon, from my heart and I feel there are many we left at your facility that are feeling a similar Thanks.

After having a stroke, Horizon Health & Subacute Center brought my dad back from the brink of death. They were the only rehab who thought he had a chance. There were times when I was not happy with the way the staff communicated with him or the family but we powered through. The business office and case managers were great. Most of the hands on staff were too including speech therapy, physical therapy and housekeeping. If you advocate for your loved one, this facility is a good choice.

My mother is currently a patient there, aside from the Covid outbreak, prior to that outbreak the staff is always hard working and polite to patients and visitors, in my opinion there should be light music playing while in the lobby area, the facility is very clean, and in a nice area of Fresno, safety wise there should be better lighting in the parking lots, overall score 10/10

My Mom was recently discharged from Horizon> We wish to commend the great rehab staff and nurses aide named Stephanie. Stephanie was so caring. But.......... The rest of the experience was not good. The food was awful and pretty much inedible. The fact that my mom ate next to nothing was never addressed and no one kept track. The wait for assistance especially for the bathroom was so long that Mom and her roommate often used their diaper which was so undignified for the patient. After getting assistance my Mom and roommate were often waiting up to 20 minutes to get back to bed.We waited 2 days for an ok for an antidiarrheal . Showers were rare and given in a very cold communal area. Mom only had 1 shower in her 7 day stay and no bed bath or hair wash etc. Its a sad state of affairs that our country has such poor nursing home care.

First time visiting my grandma who’s been at this facility for a while. From the front desk staff to the staff encountered walking to my grandma’s room were very helpful & attentive. I appreciated their help and kindness, especially it being my first time at the facility, thank you!

We transferred from Willow Creek (which was disgusting) to Horizon which was clean, and neat and the people personable and professional. The staff all seems amazing and caring and willing to do whatever it takes to take care of you and manage your pain. The food was amazing and they did everything to accommodate the food preferences and food choices.
We are so impressed with Horizon, doctors
and staff. We feel blessed to have been accepted here.
