I posted a review under Choices Healthcare of Sedona since this office appears to go by many names and identities. I have the same complaints others made. Non receptive staff, unable to obtain medical records, ongoing confusing about the name of the practice, lack of Doctor Oversight (I never once saw an MD or DO as a patient in nearly 2 years only Nicole Pangborn who does not look like woman in the photo on their website), they’ve demonstrated numerous HIPAA and PHI violations, traumatized me with their unprofessional healthcare staff, misrepresention, and lack of consistency. Moreover the referrals made by Nicole Pangborn PA-C cost me thousands of dollars in billing and medical expenses that I didn’t need as an uninsured, unemployed patient with a chronic health conditioning resulting from PTSD, through her outside referrals for labs and specialists that did nothing more than add thousands of dollars in excessive billing charges due to expensive outside medical referrals. Additionally Sherri Meltvedt RN has some atypical approaches to healthcare not only violating HIPAA laws but evading healthcare provider responsibilities to provide ethical care and protect patient rights. I hope they can correct their mistakes, they had some potential but given the years of bad reviews, who knows.

Would like to give zero stars. I saw Sandra Adair who, in these covid times, was unmasked and said I could take off my mask. I have been vaccinated and also have had 'breakthrough covid.' Unsound practice from her and a terrible way for a health care practitioner to set an example. When I called politely to complain and ask for a different practitioner in the same office, the office staff, Linda, got extremely defensive and angry, and denied me service with the obvious excuse that the other practitioner is not seeing new patients. This whole experience was awful. The staff was extremely rude and unethical. Do not go here if you value your health and politeness. The worst medical experience I have ever had, and the first review of a medical practitioner/office I have ever written.

It scared me to be in this office with unmasked caregivers, like I could be in a Delta Variant shower. The Dr. Called me John on my 4th visit, unaware of my medical history with her, making notes on another patient's file until I corrected her. I wrote her a letter telling her I understand her staff may be stressed, yet it was still frustrating. The fear drove my oxygen to dangerous levels. The oxygen tank she offered was empty and I heard her assistant asking for an emergency restocking of their supply.
This was exacerbated finding that after a year under her care, and me signing release forms from my PCP physician, she still hadn't seen my past test results/CT scans and could not monitor the progression of my primary, progressive and fatal diagnosis. As a former medical professional (nurse, pharmacy tech. EMT) I probably hoped for a more informed consultation than the 3 or 4 that I experienced.
Again, I know the pandemic surge has all medical workers stressed. That said, a lung doctor who lets her staff talk with patients like me without masks or barriers (back desk scheduling desk) scared me enough to go out of town to find a safer environment for care. I told her I'd found another Pulmonologist. I hoped she would have found a written description of my experience informative. I also noted her battery saturation meters may be uncalibrated with photos showing they measured my oxygen levels 4 to 8% lower than 2 of mine and several VVMC clinics. Today I received a discharge letter. The reason: She found my fearful reaction to her care and my written feedback abusive.

Wonderful care from this office, Nicole Pangborn and Erika Cappelluti are great and caring physicians. The medical assistants are prompt, efficient and courteous and the front desk staff friendly and helpful. Thank you all for taking such good care of my husband's and my medical needs!
