My mom was recently a rehab patient at Oak Knoll following a hospitalization. The facility was clean, neat and welcoming to enter. Front desk staff were pleasant every time I visited. My mom was cared for well during her time in rehab (3 weeks) and her needs were met. They worked hard to get her follow up appointments with outpatient providers (thanks Paula!), gave wonderful care (shout out to Naomi and Mia), and she had a positive experience working with the rehab team (thank you Connor and Rachel)! And a Big thanks to Olmy, who made my mom feel human, cared for, and safe. Whenever there was a small issue it was attended to quickly. Would recommend if looking for sub-acute rehabilitation for you or a loved one.

A family member of mine had recently stayed here and I couldn’t say nicer things about the staff. Though we were only there for a short time, each person we encountered treated my loved one and family with respect and was very helpful. I would recommend to others needing a place for a family member.

I am a Senior Companion for patient Norma Sacks. I am here four days a week for three hours. She has received good care during her stay.
The nurse Doreen is always available and I am impressed with the time she spends with her and how well she has gotten to know her family. Nurse Dorothy has gotten the knack of enticing her to take her pills. This is a dreaded task.
Aids Camilla, Kianna, Elaine, Julia and Isabell are attentive and show compassion while doing a difficult job.
Alfreda and Andrea are great thorough cleaners that always have a smile and good wishes for Norma.
Lastly, Olmy the Patient Advocate drops by 2 to 3 times a week. He’s always helpful and fun to chat with about what’s going. He is an important part of the team. The 2 to 3 times I have had frustration with care, he has calmly help me and talked me down from the anxiety I felt.
And by the way the food is wonderful!

My mom had a great stay at Oak Knoll. I just wanted to leave a review to acknowledge the excellent nurses and aides on the rehab unit who cared for her. The physical therapy department was amazing too! A special thank you to the patient advocate Olmy for checking in on my mom daily to make sure she had everything she needed. Highly recommend this spot to others!!

It is truly the staff that make a difference and the employees and staff at Oak Knoll did just that for my mother. I can say without a doubt that my mom received exceptional care from those that interacted with her on a daily basis. I spent a lot of time with my mom at Oak Knoll and witnessed first hand the care that she received, and, it went well beyond just care. There were several times when I saw a staff member interact with my mom with a genuine sense of kindness and love that exceeded a duty or obligation. I knew that my mom was being truly 'cared for' and not just being 'taken care of'. I am truly thankful for everyone at Oak Knoll.

My father had a very unfortunate experience here in March of 2022. He fell and broke his nose after his walker had been removed from his room. Only in doing my own due diligence did I find out that he had suffered a seizure the night before and when his roommate called for assistance 3 people came and "soothed him", but it was not in any way reported, it did not get him back to the hospital for further treatment, and it wasn't reported to me. The Director of Nursing wasn't even aware of it! In addition to being left wet and unaided, with aides on personal calls right outside his door, I was completely dissatisfied. I am an involved daughter and my father's care was my priority, but for all the people that don't have someone who can be that present, I fear for them in this facility. I met with the Head of Nursing to discuss my significant concerns and there was no follow-up, I asked to speak with the facilities director and he was conveniently unavailable. One star is too much for Oak Knoll, in my opinion.

Poor for those on the upper floor. Mostly just perched, sometimes uncomfortably. Given as little attention as can get away with. Nurse pushed 5 pills down throat so she could move on quick. No help with getting some water or to a radio or room or bathroom. Zero chat. Activity assist comes up but nurses say" need to medicate soon and I don't have time go down there' ..These are the ones that can't get to dining and need help the most, get the least. Hidden from the public eye.

I had a nice stay everyone was so nice and kind. However they lack in PT. Lisa the Go to person give me all the tools I needed to complete my stay. All she did was walk patients in the hallway wiyh wheelchair behind you. I am getting more help at home. I want to thank all staff members and Alamy the coordinator and the nurses Elaine,Julia,Kiera,Ozzy,Hector, Maggie. Iam so glad to be home, You'll see me one day when I am fully recovered.
