Do not take your elderly relatives here.
This has been a terrible experience. The clinic lacks accountability at every level—from management to clinical staff. I filed a complaint and submitted an incident report after a provider sent the wrong medication to my mother. To this day, no one has reached out to us. We’re still waiting for the practice manager, Raul Martinez, and Alejandra to contact us.

What’s worse, my mother’s insulin prescription was never sent. Because of that, I had to take her to the emergency room. Instead of insulin, the clinic sent antibiotics—something I reported immediately. I called multiple times and filed complaints, but no one from management has followed up.

It’s hard to believe they claim to care for elderly patients when they clearly show no concern for their health or safety. How can providers leave office visits unsigned, prescriptions unsigned, and no one seems to know anything about it?

Elderly patients often have chronic health issues that require real, attentive care—and sadly, Oak Street Health is not the place for that.

I had no problems with this clinic until recently when I needed to get referrals for my father. He has been a patient there for almost 3 years and I normally don’t have problems as they are usually nice and attentive to him. But recently they have been lacking on all areas in customer service when I requested for a referral to be sent to the insurance to be approved for a procedure my father needs to be done. I had been requesting for a month for this referral to be sent and I kept getting the run around and when I spoke to the insurance because the date of the procedure was approaching (in 3 days) they told the referral was sent 2 days ago!!! I was livid because I had been calling for weeks about this and they kept telling me it was the insurance that was delaying! I finally get the referral to be sent correctly (as so I thought) only to be told by the insurance it was denied because the doctor did not specify why the procedure was needed. I’m so over this lack of information, communication and accountability from this clinic location.

Don't trust this place to bring your elderly here and even less on their own! Quite frankly, for a local clinic specializing their "attention" to the elderly their treatment is poor. My grandma had lab at 1pm we arrived at 12:50 it's past 2:00pm and we are still here. While I'm sitting here observing others, no this receptionist not say to a patient. " well how many times do you expect me to call you!?" WOE! 😲That's just crazy, why do you speak to patients like that young lady?This place is clearly stressful amongst it's staff members, I saw an attempt to cancel AND CONCEAL their stress but that you can't hide for log. In my opinion Thai place is not a healthy circle for anyone. I just hope my GMA gives me the go to switch providers but literally don't bother with this is a negative spot.

Horrible service. 3 years in a row of problems and the Office manager after talking to her she assured me it won't happen and it happened over and over! Lab results that were supposed to be given in less than a week took three months down the line, and I had to initiate the call. The higher up administration never calls you back at all. I contacted them 2 months ago, and still to this day, they have not responded to my calls or email. DON'T GO TO OAK STREET HEALTH! YOU ARE BETTER OFF GOING TO STROGERS! AT LEAST THERE IS ACCOUNTABILITY AT STROGERS AND WHEN YOU COMPLAIN THEY LISTEN AND WORK WITH YOU! I called and same bull as always they say somone will call you and nothing it's been 2 weeks after this week it will be 3 weeks of no call back because they don't care about the patient all they care is the money!!

I had an unpleasant experience.
The waiting period for a referral takes a great amount of time, It has been 9 days since my initial appointment with the nurse practitioner and I have yet to receive my referral to consult with a specialist. When I inquired about the delay, I was told the hold up was on the insurance, however, I contacted the insurance company and I was informed that no authorization was required for an in network referral.

My mom is now a patient in here. So far, she loves how she is being treated, and the fact that they are taking care of her health is a plus. She was a patient at Access medical center, and we had a bad experience so that is the reason I wanted to try Oak Street. Everyone is so kind at Oak Street and the attention she gets is amazing. Definitely if they continue taking care of their patients they would be retaining them since our seniors need to be taken care off.

I was vaccinated for COVID-19 here (as a healthcare provider) this evening, and cannot say enough good things about the team here. In spite of a very busy evening, they were communicative and professional at every turn. My vaccine was administered by their Medical Director, Dr. Martin, and he was absolutely wonderful - engaging, competent, very friendly "bedside" (chair-side) manner, and so clearly passionate about what he does. The rest of the team, from the check-in staff to the coordinators, to the nurses, were all wonderful as well. The space was clean and well-organized. I hope this clinic thrives because it is a gem!

I was contacted byOak Street Health for the Moderna vaccine. Although I have a physician that I am happy with they were the ones that led me to the vaccine when it was very hard to find. The facility I visited was in Little Village. There is somewhat limited parking, but I was able to find a spot for both visits. The facility itself is modern and clean and staffed by friendly people who made me feel welcome. I would recommend them to anyone living in a neighborhood that has one of their offices. They will even offer transportation to people who need it to and from appointments.
