The facility & staff from admitting to rehab to long term care have all been wonderful.
I give them a 10 star if possible. The nurses & DON Kim has been awesome in efforts to get some needed thing for my dementia & high anxiety 84 year old husband.
I am concerned about 1 thing, the facility Dr Carter, I hope to feel better about soon & would give 0 stars if possible. I feel delayed needs of medication for my husband of 60+ years is causing more anxiety & could be helped with attention from the Dr to take care of this need. My husband is my top priority!

We were lucky to have diverted my 95 years old mom from Montgomery hospital to Prattville rehab. From. The Get go we were met by a warm and compassionate director that helped me deal with the stress of almost losing my mom. During the 21.days stay, the full staff from cleaners to nurses to therapist treated my mom with such a care, it almost brought tears to my eyes. All the staff cared, never ignored. My Mom and took care of all her needs. Beside the staff, the room was impeccable, clean and all Équipement was fully Functional. I could swear it was like a residence inn or a Marriott. So clean. Food quality was As described:well balanced for a 95 years old person. Finally the doctor and his assistant were very attentif and corrected wrong courses from the hospital in Montgomery. Glad. We get to go home tomorrow but I shall be forever gald I crossed path With such a crew. Word of advise:before coming in I was a scared after reading Some reviews so please know beat way to deal this complaints is to work with the very receptive team.

If I could leave a negative star review, I would. Terrible people and they lie. I was told all they needed me to do was gather paperwork for dad, but when he passed they sent the bill to me. After a hospital stay, the day after he returned to the facility, my brother went in and he was on the floor with blood all over his arm. He asked if he fell and no-one knew. Later, after administration was contacted we were told he liked to lay in the floor, however, the nurse that night did not know that. NP told me when he returned from the hospital at the end of Nov. that she had been treating him for pneumonia for two months, yet no-one had told us that and I had been a couple times, as well as my brother. Also, the amount that medicaid allows each month ($30) doesn't get returned to the family or his estate or even applied to the bill they sent. I was told they send that money to the corporate office. They are very dishonest at this facility and I should have trusted the reviews, but was told that they were under new management. Please, please read the reviews and get any and everything in writing if she choose to ignore all the reviews.

In response to the reply from the business, yes I talked to her, not sure you could call it working together since I was just told yes, sorry YOU do have to pay this. So, no it wasn't a happy resolution, if you look at the emails between us, you can see how it took many to even get them to correct it for the *right amount* and they never did give me a paid in full receipt. Then when I paid and they sent me the invoice, invoice said it wasn't that much, they said it was a misunderstanding and that what I was sent was what Medicaid owed or something. Honestly, don't want it on my credit and they know that. Maybe corporate needs to look at the emails and then get back and respond, because my thoughts are still the same and I still say be very careful sending your relatives here and get EVERYTHING in writing.

My husband stayed there for 3 weeks. Staff was wonderful. They kept me up to date. Very friendly and courteous. I would highly recommend them. They are wonderful!!

Yesterday a stranger says to me, "Come here. I need to tell you something." I had no idea what she needed to tell me, but I walked over to her. She hugged me! She told me, 'I know that you don't know me. I just felt like I wanted to hug you. Everything will be OK '
Wow!! I can still feel her love! She was just one of the amazing people who work here. They have treated my mother well in rehab and helped with recovery. I do wish admin would have communicated earlier that she wouldn't be able to stay until completely ready to be home. They did give us about a 10 day notice to make other arrangements. Overall, great choice from our experience.

IThis place shouldn’t be allowed to operate as a healthcare facility. It’s clearly a violation of all kinds of health codes. Who is managing this place. Prattville come on let’s do BETTER.

Good day, the Health & Rehab facility serves an important part in the community and we are grateful that mom had a good stay. She had a bath everyday. The dietary staff are pleasant. Shoutout to Sharon my mom's fav CNA. Therapists like Sheila are invaluable. Thank you for transporting mom to and fro dialysis.
HEADSCRATCHING - not everything is perfect especially in such an environment. Please see below.
On her 1st or second trip to dialysis she suffered a about a 3 inch laceration on the her calf while they moved her from the recliner to the wheelchair - dialysis should've properly treated this as it wasn't addressed until 2-3 days later when it was brought up to the NP on Sunday. She was transported to the ER then to have the scratch "glued" and steri strips applied. She had brought this before up to the staff , but it just wasnt taken SERIOUSLY.
EDITED - Mrs Tammy the administrator brought to my attention the nursing did follow their policy regarding these wounds. The wound was monitored per policy. It was order of provider when they made rounds to send her to the ER to get wound sufficiently closed.

Second headscratching is her last week she had loose stool (diarrhea) 4 days. It only at the 4th day after she cried that immodium was given. Again not taken SERIOUSLY.
I would give the rehab 5 stars as they overall give good service to the community, but the head scratching moments takes it down to 3.5 stars

The staff at this facility does not respond timely to call lights and sometimes don’t respond at all. My brother slept sitting up in his wheelchair on March 3rd.because his shoulder was hurting and he needed help getting in bed. He needs help getting in bed but aids are too busy to assist. His shower days are mon, wed and Friday and if I don’t go over on those days he likely will not get a shower. I called the administrator to discuss this issue and she forwarded me to a different phone. I have asked 2 different days for a pack of wipes and I was told they did not have any. how do they clean residents up if they don’t have wipes. I know everybody is short staffed but I have been extremely disappointed in this facility and the lack of care and concern for my brothers needs.
