Premier Care Center in Palm Springs is without doubt the best rehabilitation center I have encountered. The management, social workers, nurses, and rehabilitation team are dedicated and determined to improve the health of each patient. They are professional, knowledgeable, and compassionate. My mother at age 88 was a patient at Premier Care Center for 3 months. I visited the facility almost daily, and always found a friendly environment with excellent care in clean surroundings! Highly recommended by my family!

Im gonna have to go against recent reviews here off my own personal witnessing of my Grandmas stay.
About a month ago my 90 yr old Grandma was transferred from hospital to Premier, I was notified of the transfer, and came to meet my Grandma there. I arrived a few hours after the transfer and when I walked into the facility down the hall and it reeked of the smell of feces, I was taken back a bit.
I then arrived in the room which was so dark, and gloomy, windows dirty and where my Grandma (still not fully coherent due to medications from her break) was curled up in half the bed (bed was in up position) I was appalled on what I saw. I then went and had someone come adjust her. They were bringing in her food. She was on a puree diet and they of course had soggy chicken strips with carrots to eat. She can not eat this. I had to send it back.
I opened the bathroom (very small toilet sink area) not sure how someone with a wheel chair or walker can get into there. Nonetheless the bathroom reeked of urine. :/
I left for the night hoping that as long as she is cared for thats what matters, the next morning I had called to see how she was doing, I could not get anyone to pick up or return my call. Without exaggeration I must had called 8 times. I was upset and if I had lived locally I would hd been there.
With this experience I decided it was in her best interest to transfer her out and we did so. I had to have another facility call and finally get a hold of someone to set up her being transferred out.
I definitely would not recommend this place.

So my mother was at Premier Care for about 10 days. The beginning of the visit of course is always hard for patients and their visiting families. I felt I needed to be there to make sure she was taken care of and wasn't lonely so I would stop in at breakfast time and dinner time. But also I felt I wanted to monitor her eating. They give a lot of food and it wasn't bad but I wanted to make sure she had the correct diet for her issues. Not knowing things at the beginning was hard. I feel the doctor, the social worker, and the PT should set up an appointment or zoom call with the family members right away to introduce themselves, give their contact info, and let us know what is going on. I did happen to catch the doctor, nurse practitioner, and case manager once and that reassured me somewhat but never had a returned phone call when I left 2 messages. CNA's and Medication Managers were always there to help and the center was very clean.
I would like to give a special thank you to a few outstanding employees that my mother and I sincerely appreciated their help... First of all, Letty was there at the front desk with a smile each morning to greet me, at night Medication Manager Jenni's personality was so positive and was always there to help, CNA Ariana was very sweet as was Blanca too, both really cared. I also wanted to give a shout out the the PT Dept.... Jessica, LaLa, and Craig. So all in all, everyone played a part in her recovery and we thank you! My mother is doing well and almost back to normal health in her new home!

I brought my aunt to this facility a few days ago and all through I was impressed with how the nurse treated her, I was even more amazed with how clean this facility was!! I’ve had to frequent these types of facilities a lot in the past year due to the health of some family members, but this one hands down is best in my opinion. I started to assume that cleanliness just wasn’t top priority for some of these places but when we went to premier, I was clearly proven wrong! The staff there is amazing. 10/10

I want to talk about how extremely friendly the staff is, I'm always greeted with a smile. I've always felt that the people, especially the therapists, show genuine concern about my improving health and going back to a normal life. It actually is a pretty nice looking place, very clean. The housekeeping staff keeps your room tidy at all hours of the day. I wouldn't hesitate to recommend Premier Care to any of my family and friends to come here. I feel lucky to have been sent here when I originally had no intention of coming to a nursing facility. Surprisingly, the food is pretty darn good too!

My husband went to Premier skilled nursing facility after being in the hospital for a total hip replacement. Don't ever let anyone you care about go here! The manager lied to me about the room where my husband would stay. He said it was bright and cheery and it was actually gloomy and dark. It was small, not even enough room for a chair and his light did not work even when we asked numerous times to fix it. The so called hospital bed was old and you had to use a hand crank. The toilet in the shared bathroom was small and dirty. My husband was there for three days and they never changed his bed, hospital gown or cleaned the room or bathroom. One time he pulled the cord in the bathroom to get help and no one came. His room was near the nurses station so he started yelling for help and someone finally came in a hour. The food was so bad my husband couldn't eat it. He asked the nurse to cut a piece of chicken and she broke the plastic knive. I finally got my husband out of there against medical advice because he said the treatment was so bad he felt he would die there. I am not exaggerating!! Do not go to this facility!!

I'm shocked how they can maintain a 4.2 rating I was transferred here by Eisenhower Hospital about a week ago for wound care.
I'm trying my best to keep an open mind, even making excuses for their benefit, but I can't in good conscience go on pretending any longer.

I require several medications to ne given to me at certain times and they have failed miserably. The meds never come early, always late, sometimes very late, or not at all!!
Just today my roommate had to call the nurses station 5 times in 3 hours just to get a stool softener, and he was in serious pain!

My experience has not been any better either. Waiting up 2-4 hours for meds, unless they forgot and it doesn't come at all.

Today was my shower day, upon entering the shower i see this floor to ceiling, hairy black mold in the grout where the tiles have buckled. Immediately I could tell all they do is bleach it. Bleach creates a sterile clean environment so the mold grows back faster and more of it.
TSP (Trisodiumphosphate) is what professionals use to inhibit mold growth. Point being this is not a sterile environment for wound care. A few days ago I had a cockroach run across the footboard of my bed. Tried to kill it but it escaped.

The food is pretty nasty if you don't use salt, pepper, or something on it to disguise the flavor.
You get tiny cups of room temp milk, and proportions are small and usually cold. Remember to bring your own condiments if you need salt or whatever to make it more palatable, it could take you up to an hour to get it.
